About BEATRICE: Genomics For Breast Cancer Prevention in Primary Care

WE ARE CONDUCTING AN EXCITING STUDY FOR WOMEN WHO MAY BE ELIGIBLE FOR
BREAST CANCER GENETIC TESTING.

You may be eligible for genetic testing for breast cancer genes (BRCA1 or BRCA2) if you
have family members with any of the following:

1. Breast cancer before age 50
2. Cancer in both breasts
3. Breast and ovarian cancer
4. Men with breast cancer
5. Multiple cases of breast cancer
6. Ashkenazi Jewish background
7. Known BRCA1 or BRCA2 mutation

Other eligibility for the study include:

1. Woman, age 21-75 years
2. No prior breast or ovarian cancer or genetic
testing for breast cancer genes
3. Healthcare provider at Columbia University Medical
Center/NewYork Presbyterian Hospital
4. English or Spanish-speaking

As part of the study you will:

● Learn about your own breast cancer risk, receive
educational materials and give us permission to share your
breast cancer risk information with your healthcare provider

● Complete 4 brief surveys, get paid a total of up to $150 for participating

● If you decide to get genetic testing and lack insurance coverage, the study will pay for your BRCA testing.

ESTAMOS LLEVANDO A CABO UN INTERESANTE ESTUDIO PARA LAS MUJERES QUE PUEDAN SER ELEGIBLES PARA LA PRUEBA GENÉTICA DEL CÁNCER DE MAMA.

Usted puede ser elegible para las pruebas genéticas con el fi n de detectar genes del cáncer de mama
(BRCA1 o BRACA2) si usted tiene miembros de la familia con algunos de los siguientes:

1. Cáncer de mama antes de los 50 años de edad
2. Cáncer en ambas mamas
3. Cáncer de mama y ovario
4. Hombres con cáncer de mama
5. Múltiples casos de cáncer de mama
6. Antecedentes de judío asquenazi
7. Mutación conocida de BRCA1 o BRACA2

Otros criterios de elegibilidad para el estudio incluyen:

1. Mujer, de 21-75 años de edad
2. No cáncer de mama ni de ovario ni pruebas genéticas para genes de cáncer
de mama previos
3. La atiende un proveedor de cuidado de salud del Centro Médico de la
Universidad de Columbia (CUMC)/Hospital New York Presbyterian (NYPH)
4. Habla inglés o español

Como parte del estudio usted:

● Conocerá su propio riesgo de cáncer de mama, recibirá materiales educativos y nos dará permiso para compartir con su proveedor de atención de salud su información del riesgo de cáncer de mama
● Completará 4 encuestas breves, recibirá un pago total hasta de $150 por participar
● Si decide someterse a las pruebas genéticas y no tiene cobertura de seguro, el estudio le pagará su prueba de BRCA
